Community Health Reporters will provide the Center with monthly reports verifying new and existing practices and providers (General Practitioners, Family Medicine, Internal Medicine, OB/GYN, General Surgery Pediatricians, Psychiatrists and Pharmacies) hours of service, physical address, and if new patients are being accepted at these practices.
They will also be reporting any local news regarding health departments, hospitals, or ancillary medical services in the community.
Applicants must be 21 or older. Must have reliable transportation, site visits will be required quarterly. Must be comfortable with changing technology and have access to email and a cellphone. Reporting will be completed online.

Counties in GRAY are not participating at this time or are not designated as rural. Territories designated in WHITE are stand alone counties who will be assigned 1 reporter. Territories are subject to change as the program expands.
