Participant Description

Community Health Reporters will provide the Center with monthly reports verifying the following:

  • new and existing practices and providers (General Practitioners, Physicians in Family Medicine, Internal Medicine, OB/GYN, General Surgery, Pediatrics, and Psychiatry, Pharmacies, Nurse Practitioners, Physician Assistants, Dentists, Ophthalmologists, and Optometrists)
  • hours of service
  • physical address
  • if new patients are being accepted at these practices

They will also be reporting any local news regarding health departments, hospitals, surgery centers, and ancillary medical services in the community.

Community Health Reporters will receive a monthly incentive for their work and reporting responsibilities.

Qualifications

Applicants must be 21 or older. Must have reliable transportation, site visits will be required quarterly. Must be comfortable with changing technology and have access to email and a cellphone. Reporting will be completed online.
